    Please see the information below on how you or your loved ones can make a claim for our plans:

    The Guaranteed Over 50 Plan

    The Guaranteed Over 50 Plan is designed to pay a cash sum for one or more of your loved ones on your death. When the time comes for your loved ones to let us know that they’ll be making a claim, they can simply call our friendly UK based team on 08458 35 36 37 making sure that the following information can be confirmed:

    • The death has been registered and the death certificate has been received
    • The death certificate entry number and registration district
    • That they are the spouse or child of the policy holder
    • The death occurred on the UK mainland
    • The death was not as a result of something that happened whilst the policy holder was a fare paying passenger

    However, if they would rather complete the claim process in writing, then they will need to send the following to us in one go:

    • The Policy Schedule
    • The name of the person making the claim and evidence of their entitlement to the policy proceeds
    • The name and date of birth of the policy holder
    • The original death certificate
    • The account number and sort-code of the bank account where the cash sum is to be paid
    • A copy of the policyholder's Will if there is one

    This information should be sent to us at:

    It is important that you keep the policy documents safe and let your next of kin know where to find them.

    If any of these documents are not available, the person making the claim should call us on 08458 35 36 37.

     
     Recording your wishes

    If you'd like to record the names of your beneficiaries, you can complete an 'Expression of Wish' form (PDF 0.21MB) and send it to us for safekeeping. If you'd like us to send you a form in the post, call 08458 35 36 37.

    The 'Expression of Wish' form is an Adobe PDF file, which means you'll need Adobe Reader installed on your computer. Most computers will have this software installed, but if yours does not, you can download it and install it for free from http://www.adobe.com/reader (link opens a new window).     Funeral Benefit Option

    If you've selected the Funeral Benefit Option, your family will need to contact The Co-operative Funeralcare or Golden Charter directly, depending on the plan provider. This information will be available in the Welcome Pack received from either The Co-operative Funeralcare or Golden Charter.

    To make a claim through The Co-operative Funeralcare, your loved ones can either call 0845 602 0075* (open Mon-Fri 8am-6pm; Sat 9am- 1pm excluding public holidays) or call a local Co-operative Funeralcare Funeral Home to make a claim.

    The Co-operative Funeralcare will need:

    • The full name and address of the policyholder
    • The Sun Life Direct policy number
    • The Co-operative Funeralcare Funeral Planning number
    • The original death certificate


    If the Funeral Benefit Option provider is Golden Charter, then your family can call 0800 171 2077* to make a claim.
     
    To process the claim, Golden Charter will need:

    • The full name and address of the policyholder
    • The Sun Life Direct policy number
    • The Golden Charter reference number
    • The original death certificate

     
    * Calls may be monitored or recorded for security and training purposes. Call charges from mobiles may vary and you may want to check this with your service provider.

    The 50 Plus Protector Plan

    Claiming a cash sum 

    The 50 Plus Protector Plan is designed to pay a cash sum for one or more of your loved ones on your death.     • The original death certificate
    • A copy of the policyholder's Will if there is one

    Funeral Plan and Funeral Assist

    Claiming your chosen funeral services

    To make a claim your loved ones can contact The Co-operative Funeralcare directly – there is no need to contact Sun Life Direct as well. Your loved ones can either call 0845 602 0075 (open Mon-Fri 8am-6pm; Sat 9am-1pm excluding public holidays) or call a local Co-operative Funeralcare funeral home to make a claim.

    To process the claim, The Co-operative Funeralcare will need:

    • The full name and address of the policyholder
    • The AXA Wealth Ltd policy number
    • The Co-operative Funeralcare Funeral Planning number
    • The original death certificate

    If any of these documents are not available, the person making the claim should contact The Co-operative Funeralcare on 0845 602 0075.

     

    Claiming a cash sum 

    If your loved ones are claiming a cash sum instead of the funeral services, they will need to contact Sun Life Direct rather than The Co-operative Funeralcare to make a claim. They can simply call our friendly UK based team on 08458 35 36 37 or write to us at Sun Life Direct, PO Box 446, Bristol, BS99 1DR.

    To process the claim, we will need:

    • The Policy Schedule to be returned
    • The name of the person making the claim and evidence of their entitlement to the Policy proceeds
    • The name and date of birth of the Policyholder
    • The original death certificate
    • A copy of the Policyholder's Will if there is one

    In the event of death, we will need:

    Terminal illness claims:
